Types of Payment Accepted
- We accept personal checks.
- We accept credit cards – Visa®, MasterCard®, American Express®, and Discover®.
- For our patients with no insurance, we do collect a minimum of $200.00 up-front as a deposit to be used towards the visit cost.
- There will be a No Show / Late Cancellation fee of $75.00 for any appointments without a 24 hour notice.
Insurance Plans
We participate with most insurance companies, a few of the most common routine vision vendors we accept are VSP, Northwest Administrators (NBN). We are also providers for Medicare, Regence, Premera, First Choice, Aetna, Group Health, United Healthcare, and more. Please call our office for more details or better yet, contact your insurance company directly to verify if we are providers for your plan. Please be sure to obtain any necessary referrals from your primary care physician prior to being seen in our office, generally a referral is only required for medical services and not routine health exams.

Facilities and Equipment
In-Office diagnostic equipment including autorefractor, corneal topography, scanning laser ophthalmoscopy, digital fundus photography, automated visual field, digital slit lamp, ultrasound pachymetry, scanning laser retinal tomography, Argos, optical coherence tomography (HD-OCT).
Languages Spoken
Our staff speaks English, Cantonese, Mandarin Chinese, and Spanish. You are responsible to provide your own interpreter, our technicians are not able to perform their professional duties as well as interpret fully for your appointments.
